0345 (3) Licensor observed lapses of supervison because of the lay out of the childcare home Correction: Provider created a supervison plan that only two areas will be open/available at the same time to ensure supervison requirements are met. 0345 (5)(d) It was observed that provider left a child for a minute or two in the diaper changing station to help another child.
